Bisphosphonates, Osteonecrosis of the Jaw, and Your Dentist

Today, I have a gargantuan share of mail from Novartis, the drug manufacturer. It is not a marketing share like so mighty of the mail dentists receive. It is a nine by twelve envelope with several pages of information about Bisphosphonate associated Osteonecrosis of the Jaw or BON and their bisphosphonate medications, Zometa and Aredia. Thanks to the lay press and the most recent issue of The Journal of the American Dental Association, I am already aware of the issue. 

Bisphosphonates such as Fosamax, Aredia, Zometa, Boneva, and Actonel are a godsend to people who suffer from diseases that effect bone density. In healthy individuals, bone is in a constant cycle of breakdown and rebuilding. Bisphosphonates prevent the breakdown of bone. Imagine how useful that is to a person with osteoporosis, a well-liked disease that weakens bones. Bisphosphonates are very effective in the treatment of Padget's disease of bone and hypercalcemia (increased calcium in the blood) caused by some cancers. Unfortunately, bisphosphonate-associated osteonecrosis of the jaw (BON) is a painful complication that affects some patients who take bisphosphonates.

BON is not a new affliction. Recently, Ruggiero et. al. enumerated other risk factors for osteonecrosis in the Journal of Oncology Practice (volume 2 issue 1). Widely varied risk factors include conditions - malnutrition, advanced age and edentulous (toothless) regions, diseases – cancer, osteoporosis, Gaucher's disease, systemic lupus erythematosis, and sickle cell disease, substances – alcohol and tobacco, and medications – corticosteroids, chemotherapy and immunotherapy.

Many medications have side effects which the health care establishment call risks. We take the medication when the benefits outweigh the risks. Antihistamines like Benedryl (diphenhydramine) and Dimetapp (bropheniramine) relieve allergy symptoms better than other medications. That is their benefit. The risk is that antihistamines make the user sleepy. Most people who use these medications believe that the risk of being inattentive is worth the benefit of less sneezing, less itching and less watery eyes. What happens when the benefits are great, the risks are great and the medication is popular?

Bisphosphonates are in medical use since 1968. However, the first report of a link between BON and bisphosphonates only dates back to 2003. These reports are only impartial beginning to filter down to the public. Popular newspapers and web sites report the sad outcomes of patients who suffer osteonecrosis after taking bisphosphonates and the danger to the industry of numerous lawsuits on the horizon.

People who take bisphosphonates and maintain that they may have osteonecrosis of the jaw should see a dentist at the earliest possible time for evaluation and treatment. Osteonecrosis of the jaw may come on slowly or suddenly. The first symptom may be as composed as the loosening of a tooth or teeth, numbness or tingling. Most dentists learn about osteonecrosis in dental school. Osteonecrosis occurs with radiation treatments to the jaws. Dentists learn how to diagnose, prevent, and treat osteonecrosis. Dentists are becoming aware of the link between bisphosphonates and osteonecrosis along with the general public and thanks to the American Dental Association News such as “Osteonecrosis Warning” posted on May 12, 2005, on www.ADA.org.

Treatment of osteonecrosis may begin with oral and local antibiotics and antimicrobial mouth rinses. Surgical treatment may include gently removing the exposed, dead pieces of bone from the affected site. It may be necessary to tranquil exposed bone to prevent damage to soft tissue. Dentists are able to cover the affected bone with a simple removable device. Stopping bisphosphonate therapy does not seem to be an effective treatment, because bisphosphonates remain in the body long after. Patients who have BON should see their dentist every three months to monitor the progress of the disease.

Undoubtedly, prevention is the best medicine when taking bisphosphonates. The American Dental Association maintains that patients who will begins bisphosphonate therapy should consult their dentist first. The dentist should grasp teeth and provide any surgical treatment in time for healing to occur before the patient receives their first dose of bisphosphonate. The patient should begin therapy with healthy teeth and gums. Dentures should fit well. The patient should brush and floss well to prevent further dental disease.

During bisphosphonate therapy, the patient should watch their dentist ever three to four months for professional dental cleaning and examination. Treat dental infections as speedy as possible with antibiotics and root canal therapy. Avoid surgery and extractions whenever possible. As always, discuss your concerns and your condition with your dentist and your physician.

Having been experiencing pain and spasms in my left eye, I recently scheduled an eye exam to see if my prescription for corrective lenses needed adjustment. Fortunately that was the case, and hopefully new glasses and a change in lenses will correct the problem.

Easy Ways to Lose Weight

I lost weight not that long ago. I didn't have some magical potion to help me do it nor did a genie jump out of a lamp and grant me three wishes. I did it through a combination of managing my eating habits and exercise. It really wasn't all that difficult. And you can do it too. Here's what I did:

Ate more frequently

If you skip breakfast and/or lunch, you body gets confused. In an peril to protect itself from starvation, it saves its resources and stores bulky and all those calories you consumed during your last meal. As a result, you put on weight. To combat this, I ate smaller portions throughout the day to stimulate my metabolism. I never felt hungry if I ate a small portion because I was eating so frequently and it helped me to lose weight.

Consistently walked

You can put on a video with one of those perky twenty-something year olds with perfect bodies who like to do aerobics for hours on end, ultimately torching you. I wouldn't recommend it though. A steady routine of walking did the trick for me and I got to enjoy the outdoors while I did it. A couple things to keep in mind to make your walk more effective:

-Rotating between slow walking for a few a minutes, and bursts of very quick walking for a minute or two, helps burn more calories.

-Consistently walk from heal to toe to tighten thigh and calf muscles.

-Swing your arms while swaying your waist during your walk. It helps burn more calories and tightens your tummy.

-Set a goal of how many miles you'd like to walk in a month. Every time you hotfoot, write the number down on that day on your calendar to help you meet your goals.

-Try a different walking routine every so often to keep from getting bored.

Combined light exercise into my routine

I am in no way a big fan of utilize routines where I am tripping ever my fill to feet because the routines are so complicated or so ambitious that I am out of breath in the first five minutes. So I found a nice in between exercise routine. "Poke the Walk" with Leslie Sansone is an in-home walking program that combines just a few simple steps and sets them to upbeat inspirational music. Leslie really helped to motivate me. I'd walk outside for a few days and then throw in "Walk the Walk" for a day or two. The simple steps helped to tighten my muscles and shape up while adding something different to my routine. And the routine was particularly advantageous for rainy and cold days when bad weather was an excuse not to exercise. With "Dart the Walk," I had no excuse not to do it.

Cut unnecessary eating

This is going to sound certain, but I cut out things I really didn't need to eat. I realized that I was eating things when I wasn't hungry, just to eat - I am sure we're all guilty of that from time to time. So, I cut certain things and saved my calories for things I really had to have which usually involves chocolate! For example, I began drinking water at all meals to place calories (besides it's good for you) and lop bread and rolls from my dinner. I never deprived myself of anything I wanted - that would be too unrealistic. If I knew I was going out with the girls for a nice titanic meal at Carrabba's and was going to have wine, I'd have a 140 calorie Lean Cuisine meal for lunch with an apple and water to fill me. This way I didn't have to worry about the large amount of calories I'd eat at night, I still wouldn't go over my goal limit of 1,500 calories and I wouldn't starve myself.

Weighed in everyday

You don't have to go to Weight Watchers to weight yourself everyday. Simply get on a scale every morning and take note of your current weight. If it's less than yesterday, great. If it's the same as yesterday, great. But if it's a pound or two more than yesterday, you know you have to exercise more today and watch your calories. If you weigh yourself everyday, you should be able to manage weight find a little better. After all, you can't earn 10 pounds in one day. This method you if you sight yourself gaining a little, you can nip it in the bud right away.

Reminded myself that I wasn't alone

I tried to quit certain when I was trying to lose weight, reminding myself that I wasn't the only one who was trying to lose weight and that if others could do it, so could I. Because of prescription medicine and a thyroid problem, I've put back on some pounds, and I plan on trying to lose weight again. This is the plan I am going to follow. So remember, we're in this together and if I could do it before, I can do it again, and so you can you!
